4, birth certificate, a copy of each; (female employees with children provide)
5, a record the original outpatient or hospitalization;

Four, maternity insurance declaration process:
One month after the 1, fertility treatments, timely birth insurance required to declare material submitted to the human resources department;
2, 1 month and 5 days after declaration, the declaration for maternity insurance data and passbook to 10 month - 15 days to the human resources department.
